One of the more challenging sections of the APPIC is Section 3, Doctoral Practicum Documentation. In this section you will be required to report your assessment and intervention hours.

In order to help you keep organized, we have provided two Microsoft Excel spread sheets for you to download for free; an Assessment Spreadsheet and a Therapy Spreadsheet. When you download them do not "disable macros" if your program asks you. The spread sheets are set up so you just have to insert numbers in the right places and it will give you totals. Feel free to modify the cells and the formulas to suit your particular needs. While there are more complex spread sheets available, I suggest you trust you own math over anyone elses.

Therapy Logs

Section 3, question 1 (a-g) of the APPI requires you to report the total number of "face to face" hours you spent with patients based on type of therapy delivered (individual, career, group), patient age, and masters vs. doctoral work. While this therapy template will not do the math for you, it will help to keep you organized while you add up your hours. Information from this template can also be used to answer question 2 - Support Activities and question 3 - Supervision Recieved.

To download, do a right click HERE, save target as, and then select where you want to save it on your hard drive. After you download the file, open Excel and then open the file from your hard drive.

Assessment Logs

This template will help you to answer question 1h - Psychological Assessment Experience and question 2 - Support Activities (add these hours to your therapy support hours). If you list the tests that you administered to each patient in the "Tests" column it will make it much easier to complete Section 4 - Test Administration. Cut and paste information (lists of tests) in the cell to save time.

To download, do a right click HERE, save target as, and then select where you want to save it on your hard drive. After you download the file, open Excel and then open the file from your hard drive.

Hours Calculator

We often find that students do not keep careful records of the therapy and assessment cases they have had during their graduate career. In such cases, students will need to make an estimate of their hours to complete the APPI. To assist you in this task, we have created an "hours calculator" for your use. By changing the amount of time spent in face-to-face and support activities and the number of cases in each category, your hours will be calculated for you. Feel free to change this Excel spreadsheet and pass it along to others.

To download, do a right click HERE, save target as, and then select where you want to save it on your hard drive. After you download the file, open Excel and then open the file from your hard drive.
